If you have been diagnosed with coeliac disease you are entitled to get the above bread mixes on prescription.
To help you get started simply download the Prescription Form, print it off, then fill it in and take it to your GP
Glebe Farm offers a Starter Pack of samples. Please ask your Dietitian to contact us for a Starter Pack Request Card. Simply complete your details, return it to us and we'll send you your complimentary starter pack.
Once you have received your starter pack and decided if you would like one of both of the mixes, you can then set up a repeat prescription with your GP, which can be collected from your pharmacy. If you want to change an item or add something new to your prescription, see your GP and ask them to make the necessary change.
